The crystals irritate the epithelium, causing pain and discharge; flakes can ulcerate deeply with rupture risk. Treatment combines topical antibiotics, EDTA chelation, possible burr debridement in early stages, and surgery if necessary despite anesthetic risks.

Severely affected at presentation, she responded rapidly to topical immunosuppressants (steroids and tacrolimus), with a big difference in just 10 days. At her 1‑month recheck, all corneal vessels had vanished, steroids were stopped, and she’s maintained on tacrolimus alone. Ongoing monitoring is needed for flare‑ups, though most dogs remain well‑controlled on low‑dose therapy.

An elderly dog treated medically for a deep corneal ulcer that worsened in 48 h to an infected descemetocele underwent keratectomy and conjunctival graft for tectonic support and vascularization. Over 6 weeks of follow‑ups—initial exam, 48 h later, 1 week, 3 weeks, and 6 weeks post‑op—the cornea stabilized, the graft remains, and the dog is comfortable, visual, and back to normal.
